Exploring Long-Term Planning (20 minutes) • Face-to-Face: o Review the critical components of a long-term care plan: current needs assessment, legal and financial considerations, healthcare planning, social and emotional support, and independent living skills. o Discuss flexibility and periodic updates in the plan as a child’s needs evolve. o Use a flip chart to highlight each component. • Online: o Share a slide presentation or document to review the components of a long- term care plan. o Discuss each aspect in-depth while encouraging participants to ask questions via the chat or audio. o Use an interactive whiteboard to summarize key points for visual learners. 5. Activity 2: Develop a Long-Term Plan (30 minutes) • Face-to-Face: o Provide participants with long-term planning templates and have them work individually to fill them out. o Encourage them to identify resources and services for each component of the plan. o Facilitators should circulate to offer individual support as needed. • Online: o Share the long-term planning template with participants via a link or shared folder (Google Drive, Dropbox, etc.). o Ask participants to fill it out individually, ensuring they consider all components.
